Services for Students with Disabilities

UC San Diego Division of Extended Studies strives to provide equal access and an exceptional learning environment for all students. In accordance with Section 504 of the Rehabilitation Act of 1973 and with the Americans with Disabilities Act of 1990, we provide support services and reasonable accommodations based on your disability-related needs.

Our office assists students who are enrolled in individual courses, certificate programs and Concurrent Enrollment courses. We handle all correspondence and documentation in confidence.

How to Request Accommodations and Services

Please submit your request and appropriate documentation at least 20 business days before the quarter begins to allow us time to review, approve and arrange your accommodations. If your course has already begun, you may still begin the process, but please be aware of the timeline to arrange accommodations. Accommodations will take effect when the process is complete and are not retroactive to the start date of the course.

New students:

  1. Complete the Disability Accommodation Request Form.
  2. Complete and return the appropriate Verification of Disability form.
  3. Contact the Disability Coordinator to confirm your request form and verification documentation have been received.
  4. Schedule an intake appointment with the Disability Coordinator.

Continuing students: Contact the Disability Coordinator at least 20 business days before the quarter begins to request accommodations for each course. Accommodations do not automatically transfer from quarter to quarter. They must be requested for each individual course.

Depending on the nature of your disability, you may be required to submit updated documentation describing your current functional limitations.

Verification of Disability

In order to identify which reasonable accommodations we can provide, you must provide current medical documentation that verifies your disability and describes your current functional limitations.

Choose the Verification of Disability form below that most closely describes your disability. If applicable, download the corresponding UC Practices for Documentation file for your disability and send it with your form to your healthcare provider.

If you have co-existing conditions, please email unex-ssd@ucsd.edu for assistance in choosing the most appropriate form.

Once we receive your completed documentation, Division of Extended Studies' Disability Coordinator will contact you within 1-2 business days to discuss your accommodations request. Based on the documentation you can provide, we will make a determination as to which reasonable accommodations and/or services we can approve. If the documentation you provide is not sufficient, we will be unable to approve any accommodations and/or services.

If you do not receive confirmation that your form and documentation have been received by the Disability Coordinator within 1-2 business days, please call (858) 246-1032 or send an email to unex-ssd@ucsd.edu. Please understand that it is your responsibility to confirm that we have received your forms.

Americans with Disabilities Act (ADA) Compliance

Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990 prohibit discrimination against individuals on the basis of their disabilities. According to these laws, no otherwise qualified individual with a disability shall, solely by reason of his/her disability, be excluded from the participation in, be denied the benefits of, or be subjected to discrimination under any program or activity of a public entity.

If you have questions about student-related nondiscrimination policies or concerns about possible discrimination or harassment, you should contact the Office for the Prevention of Harassment and Discrimination (OPHD) at (858) 534-8298, ophd@ucsd.edu or reportbias.ucsd.edu. Students are encouraged to seek assistance as soon as possible, as time limits may apply to complaint-resolution processes.

View or download the UC Guidelines Applying to Nondiscrimination on the Basis of Disability for more information.
